Serum Prolactin Levels and Paranoid Psychosis

Abstract
This study was designed to investigate the serum prolactin (PRL) levels before and during treatment with antipsychotic drugs to patients with paranoid psychoses by the radioimmunoassay technique to see if there was any significant relation with the clinical responses. The subjects were 40 cases of paranoid psychotics (18 schizophrenics, 22 schizophreniform psychotics) whose diagnoses were made in accordance with the criteria of DSM-¥² (1979) and Hamilton¡¯s criteria for paranoia (1969). Control group were 29 cases of nonparanoid psychoses (13 schizophrenics, 16 schizophreniform psychotics) and 14 cases of nonpsychosis. All the subjects studied were confirmed to be free from organic as well as endocrinologic illnesses.
The results were as follows;
1. Mean serum PRL levels of both experimental and control group were within normal range before the treatment and there was no significant difference between the groups.
2. As for the relationship between clinical response to the treatment and change of serum PRL levels, there was significant increment of serum PRL level with improvement of clinical symptoms as compared to the pretreatment period in both paranoid and nonparanoid psychotics (p<.05, p<.01). However no significant difference was found between paranoid and nonparanoid groups. There was also significant increment of serum PRL in both male and female groups (p<.01).
3. Serum PRL levels at the point of first detection of clinical improvement was 30 ng/ml or above in the male group and 70 ng/ml or above in the female group, thus showing increased levels in both groups.
The findings of our study suggested that it would be difficult to distinguish paranoid psychoses from nonparanoid psychoses by measuring serum PRL levels, while serum PRL level would be an indirect index in finding the effectiveness of treatment including administration of antipsychotic drugs.
